All newly registered patients are required to complete a health questionnaire in full. You may be asked to book an appointment for a health check following receipt of this questionnaire. This health check gives us the opportunity to meet you and your family and plan any special health needs you may have and, where there is a family history of chronic disease eg diabetes, heart disease, conduct screening tests.

Please let us know if you change your address or telephone number. Should your move take you outside the practice area, you will need to register with a practice in your new area.Please also ensure that we have your mobile number.

This will help us greatly with the text messaging service we are developing.
